1. Platform Usage
HealthCharts is a healthcare information management platform designed for practitioners, hospitals, clinics, patients, and authorized family members. The platform allows secure management of:
- Electronic medical records
- Clinical notes and timelines
- Biometric monitoring
- Voice dictation and transcription
- Patient-family communication workflows
- Medical documentation and uploads
- Hospital-level practitioner collaboration
2. User Responsibilities
- You are responsible for maintaining the confidentiality of your account.
- You may not share login credentials.
- All medical information entered must be accurate, lawful, and clinically relevant.
- Users may not upload malicious, illegal, fraudulent, or unauthorized content.
- Practitioners must comply with HPCSA, BHF, and applicable healthcare regulations.
- Family access must only be granted through lawful patient approval or verified Power of Attorney processes.
3. Medical Disclaimer
HealthCharts provides infrastructure for healthcare information management and communication workflows. HealthCharts does NOT provide medical diagnoses, treatment decisions, emergency services, or clinical judgment. All medical decisions remain solely the responsibility of qualified healthcare practitioners.
4. POPIA & Privacy Compliance
HealthCharts processes personal and medical information in accordance with South Africa's Protection of Personal Information Act (POPIA). Information may include:
- Personal identity information
- Medical records and diagnoses
- Biometrics and clinical monitoring data
- Voice recordings and transcripts
- Laboratory results and referrals
- Hospital and practitioner interactions
Users consent to secure processing, storage, and controlled sharing of information according to system permissions.
5. Security & Access Control
- All system activity may be audit logged.
- Hospital-level segregation is enforced.
- Role-based access restrictions apply.
- Unauthorized access attempts are prohibited.
- HealthCharts reserves the right to suspend accounts involved in suspicious activity.
- Users are responsible for securing their own devices and passwords.
6. Voice Dictation & AI Transcription
Voice dictation and AI transcription features are provided to assist clinical workflows. AI-generated transcripts may contain errors or inaccuracies and MUST be reviewed by practitioners before use in patient care or medical records. Practitioners remain fully responsible for all approved clinical content.
7. Limitation of Liability
HealthCharts, its operators, developers, affiliates, and partners shall not be held liable for:
- Clinical decisions or patient outcomes
- Improper practitioner usage
- Data entered incorrectly by users
- Third-party service outages
- Temporary interruptions or downtime
- Indirect, incidental, or consequential damages
8. Account Suspension & Termination
HealthCharts reserves the right to:
- Suspend accounts violating these terms
- Remove unlawful or harmful content
- Restrict access during investigations
- Terminate accounts involved in fraud, misuse, or unauthorized access
9. Changes To Terms
HealthCharts may update these Terms & Conditions periodically. Continued platform usage after updates constitutes acceptance of revised terms.
